Understanding Sexual Performance Anxiety
Sexual performance anxiety is pervasive anxiety about sexual performance — anxiety about erections, about reaching orgasm, about satisfying a partner, about body image, about duration, or simply about “doing it right”. This anxiety actively interferes with sexual response: it activates the sympathetic nervous system (the “fight or flight” response), which directly inhibits the parasympathetic activity (the “rest and digest” response) required for arousal, erection, and orgasm.
The result is that anxiety creates the very outcome it fears. A man anxious about losing his erection activates the physiological response that causes erection loss. A woman anxious about not becoming aroused activates the response that prevents arousal. Performance anxiety thus becomes self-fulfilling — and each negative experience increases anxiety for the next.
Highly treatable: Sexual performance anxiety responds exceptionally well to specialist psychosexual therapy. Breaking the anxiety-performance cycle is one of the most reliably achievable outcomes in psychosexual practice.

What our clients say
Our Therapists
Our COSRT-accredited therapists have specialist experience treating sexual performance anxiety — one of the most common presentations in psychosexual practice. They bring specific knowledge of the anxiety-arousal relationship, the sensate focus programme, and the cognitive approaches that most effectively break the performance anxiety cycle.
Sexual performance anxiety is one of those presentations where understanding the mechanism is itself half the treatment. Our therapists explain clearly and accessibly what is happening physiologically — why anxiety creates the outcomes it fears — and then provide a structured programme that interrupts the cycle with consistent success.
Your First Session
Your first session is a warm, confidential conversation about your experience — what you have been noticing, how long it has been affecting you, and what you hope to achieve. There is no pressure and no predetermined agenda. Your therapist will listen carefully and develop a personalised treatment plan collaboratively with you.
Between Sessions
Psychosexual therapy involves work between sessions as well as within them. Your therapist will discuss any tasks, exercises, or reflections that may support your progress. These are always agreed collaboratively and tailored entirely to your situation and pace.

Frequently Asked Questions
1. Is performance anxiety more common in men?
Performance anxiety affects both men and women, though it presents differently. In men it most often involves erectile concerns; in women it most often involves difficulty with arousal or orgasm. Both respond well to specialist psychosexual therapy.
2. Can performance anxiety cause permanent erectile dysfunction?
Performance anxiety causes functional erectile difficulties that are maintained psychologically. They are not the same as organic erectile dysfunction and do not cause permanent physical changes. Psychosexual therapy is highly effective.
3. What if I have had one bad experience?
A single experience of erectile difficulty or inability to orgasm is normal and does not predict a pattern. If that experience has generated anxiety that is beginning to create a cycle, early intervention is very effective.
4. How many sessions will I need?
Performance anxiety typically responds relatively quickly — often within 8-12 sessions. More complex or long-established anxiety may take longer. Your therapist will review progress regularly.
